A 27-year-old male is brought to the ED by paramedics after being found unresponsive at home. He has a known history of recreational drug use, and empty blister packs of opioids were found nearby. His GCS is 5, and his vitals show RR of 6 b/min, HR of 52 bpm, BP of 98/58 mmHg, and SpO2 of 84% on room air. On examination, his pupils are pinpoint and non-reactive to light. You apply a 15 L non-rebreathing face mask and administer naloxone 400 mcg IV with no improvement in the patient’s GCS or respiratory status. A further 800 mcg of naloxone is given, with minimal response. His RR remains low, and there is not much improvement in his conscious level.
